Programs: Foreclosure/Default Counseling
Thank you for taking the time to reach out to RDAC. You have
taken the first step. RDAC knows this is a rough time for you and your
family. Here is what to expect:
Foreclosure/Default
Counseling is designed to empower a homeowner to make the best possible decision
based on his/her personal financial situation. RDAC’s counselors cannot
make a decision for you. Each homeowner has his/her own personal
financial situation and an individual hardship.
The first step of the process
is to obtain an intake packet. This packet contains seven documents, listed below:
Cover Letter: Outlines specific documentation you will need to
locate and copy for review. These documents must be included in the
package you submit to RDAC. RDAC staff will request missing documents
only twice before closing the file.
Homeowner Information
Worksheet: This form is nine pages
long and contains key information for assessing your situation. There is
also a budget included in this form. Please be as honest as possible and
only complete the current section. RDAC’s Housing Counselors will discuss
the adjusted and crisis areas with you where necessary.
Instructions for Request
for Modification Affidavit: Follow these instructions to
complete the form.
Request for Modification
Affidavit: This form was
introduced on 11/23/09. This form is very important and must be complete, be
truthful and honest. It will be helpful to you to complete the
Homeowner Worksheet prior to this form, in order to be sure of monthly expenses. Make
sure you explain in your own words your personal financial situation in
the explanation section. It must be complete, signed and dated.
Authorization of Release
of Information: Must be signed by all
names listed on the mortgage. Allows RDAC to speak to and obtain
information from your bank/servicer to better assist you.
Counselor/Client Agreement: Must be signed by all members participating in the
program. Explains what you can expect from RDAC’s Housing Counselors and
what they expect from you.
IRS form 4506-T EZ: This form is required by most banks/servicers.
This form allows your bank/servicer to obtain a transcript of your tax return.

It is important to return all
documentation together and complete. RDAC will not look at your file
until all the documents are received. RDAC staff will request any missing
documents only twice before closing your file.
Once you
download and complete all these documents and gather all other documentation,
you can fax, email or mail. All documents should be emailed to mortgage@ocrdac.org, faxed to
(845) 524-4665 or mailed to PO Box 1224 Pine Bush, NY 12566. You may drop
off the documents in person; however housing counselors will not be able to see
you on the spot. Do not send documents or questions to any other email, use mortgage@ocrdac.org only. After
all your documentation is obtained, RDAC staff will work with you to determine
the best solution. This can be done in person at RDAC’s office, by
telephone or through email.
RDAC is
a non-profit, HUD approved counseling agency and will not charge for
Foreclosure/Default Counseling. There may be fees assessed for other
areas within RDAC.

are other families who need help as much as you do. It is quicker to
